Address owner feedback: Add critical database table name change section

- Add Step 2: Handle Database Table Name Change (CRITICAL)
- Provide two migration options: rename table or override in model
- Renumber all subsequent steps (13 total steps)
- Update overview, breaking changes, and conclusion sections
- Maintain all previous fixes (CSS import, command names, version requirements)
