Problem description

Save form and Go to prompt icons are visible although all options are unchecked in Form Entry Settings

Steps to reproduce the problem

1. All options in Form Entry Settings are unchecked
2. User starts form
3. User sees 'Save form' and 'Go to prompt' icons

Expected behavior

'Save form' and 'Go to prompt' icons are not visible
