Similar to prod GEAR, add a "Give feedback" button to GEAR3.1. As discussed, try to incorporate it on the right side as it appears in prod, but if this is too time intensive please put it to the left of the "About GEAR" button at the top right.

Please add Google Analytics to the button so that we can track how many users click it versus how many users actually submit the form.
