💡 Optimize your view! Keep sEMG, TOF, tEMG, and tEMG waterfall views on the same screen. This will cut down on any confusion and be informative to you, your Oversight Physician, and Surgeon. #IONM #Neurophysiology #BestPatientCare

In some procedures such as CEAs or aneurysm clippings, your Oversight Physician will need to compare, side-by-side, baseline EEG and current EEG (pre- and post-clamping in a CEA, for example). This can be done by using the Look Back function. In Cadwell: ✅ Hover above the timeline located at the bottom of the screen. Select a time. During a CEA procedure, we'd suggest lookback traces around time of baseline or following heparinization, before clamp is placed. Talk it over with your Oversight Physician to confirm preference. ✅ Once a previous time is selected, you'll notice the timeline at the bottom of the screen will appear yellow and a status notification will confirm live or look back is enabled. Lookback traces will now appear in SSEP and EEG modalities for comparison. ✅ When you no longer have need for lookback feature, simply click the playback menu button to move data to current/real time.

🔍 What should clean tEMG responses look like? Like this 👇 The stimulus is applied and a clear, consistent CMAP response appears in the same channel and stays there until the stimulus is decreased. #IONM #Neurophysiology #MonitoringLife

⚡️ Because sEMG and tEMG use the same muscles and the same electrodes, it’s important to make sure the sEMG is quiet before testing screws with tEMG. If the sEMG is actively firing, the tEMG data becomes less reliable. The electrodes will essentially be “competing for screen time,” making it harder to tell whether a response is from the stimulation or just natural irritation firing. If you find yourself in this situation, consider communicating to the Surgeon something like: "I’m ready to test screws, but right now we’re seeing large-amplitude firing in the right hand. That will make screw testing less reliable. Let’s give it a minute or two to calm down, and then I can give you cleaner, more reliable screw data." #Communication #Neurosugery #IONM #Neurophysiology
