New data shows patients with type 2 diabetes face double the risk of developing sepsis. This underscores a critical need for better tools that can manage chronic conditions and spot warning signs before they escalate. Remote patient monitoring (RPM) is one of them. By tracking vital signs like blood glucose, weight, blood pressure, and temperature in real time, RPM gives providers the chance to intervene early, whether it's about adjusting diabetes treatment or responding to the initial symptoms of infection that could lead to sepsis. While it may not be a cure, RPM is a powerful means of avoiding health complications. When paired with care models like Chronic Care Management (CCM), RPM becomes part of a smarter, more responsive system. Learn more about sepsis risk and how HCRS supports diabetes management in the comments.

Not every chronic care challenge can be solved face-to-face. That’s why more providers are turning to remote patient monitoring to stay connected with high-risk patients, especially between visits. Successful RPM programs combine clinical oversight, real-time data, and monthly reimbursement opportunities, and are highly beneficial to patients who are dealing with multiple chronic conditions, recovering from hospitalization, or struggling with therapy adherence. If you want better health outcomes, reduced readmissions, and predictable revenue, then we encourage you to learn more about why RPM is an excellent solution (link in comments). #RemotePatientMonitoring #ChronicCare #ValueBasedCare Brenda Doles

For providers who are considering whether or not to implement RPM, let us ask you this: Would you rather rely on traditional fee-for-service models that keep you chained to in-office visits, or receive recurring reimbursements month after month? RPM is a powerful tool because it doesn't require patients to see you in person in order to be evaluated, receive treatment advice, and improve their own health. As long as you can review patient data and submit the appropriate reimbursement codes for time spent on those reviews, RPM can benefit your care and be lucrative for your practice. If managing this work in-house is too difficult or time-consuming, consider outsourcing it to HCRS.

Physicians: It’s time to face the hard truth! Many practices are slow to adopt Remote Patient Monitoring (RPM)—not because the technology isn’t ready, but because the mindset isn’t. The biggest roadblock? Fear of lost revenue, more labor, more work. Here’s the irony: Traditional fee-for-service models keep you chained to in-office visits. When patients miss appointments, you lose income. Meanwhile, other practices embracing RPM are capturing recurring reimbursements month after month—building stability instead of relying on no-show–dependent revenue streams. The data is clear: RPM and Chronic Care Management (CCM) add new revenue channels without requiring more physical office hours. They strengthen patient engagement, reduce ER visits, and create predictable monthly cash flow. So why wait? Because change feels uncomfortable. But here’s the truth—medicine is going virtual, whether you’re ready or not. · Stop fearing lost revenue. · Start embracing new income streams. · Reinvent your practice and step confidently into the future of care. What are the biggest health needs of your patients right now? Are they managing multiple chronic conditions? Struggling with therapy adherence? Recently discharged and at risk of readmission? These are the moments when care continuity matters most. Remote patient monitoring (RPM) allows healthcare teams to support patients in between visits by tracking vitals, symptoms, and therapy progress in real time. When paired with programs like CCM, RTM, or TCM, this system becomes a powerful tool for improving health outcomes. Learn how it all works, as well as the programs that we offer (link in the comments). Brenda Doles

What's the difference between an RPM program that works and one that doesn't? "It’s not the flashy gadgets or the slick app. It’s focus. The programs that work target the right people—the high-risk patients who are always one step away from the hospital. They have a system that flags a real problem in real time. And, crucially, they have a clinical team that can jump on it. Immediately. Without that last piece, it’s all just useless data on a screen." https://lnkd.in/eS2mmKBb We're the team that can jump on it: https://lnkd.in/eHF6CF-f

Many think that the biggest challenges with implementing an RPM program are reimbursements and device management. But there may be more to the story. What about: • Staff who feel overwhelmed by one more platform? • Disconnected EHRs that lead to fragmented care? • Patients who aren’t sure how to use their RPM tools? • Confusion around who’s responsible for follow-up? We take a broader look at other obstacles that may be holding providers back — and how organizations can confidently overcome them. https://lnkd.in/ewVi8d7W #RemotePatientMonitoring #HealthIT #CareCoordination #ChronicCare #DigitalHealth

RPM can boost revenue and keep patients healthier at home — so why aren’t more providers jumping in? 🤔 Too often it comes down to: · Missed reimbursements (wrong CPT codes) · Staff overwhelmed by data + tech That’s why HCRS built a turnkey RPM program — we manage the details so providers can focus on care. What’s the biggest barrier you see to adopting RPM?
In spite of its numerous benefits, remote patient monitoring still isn't gaining traction with many practices. Why? Two reasons are likely to blame: inadequate reimbursements, and difficulty with understanding the technology. If you aren't using the right CPT codes, you won't be reimbursed for the work you've provided. If your staff is overwhelmed with organizing all of that data collected on patient vitals, or teaching patients how to maintain their RPM devices, then they may not be as inclined to start a program. We can help on both counts. HCRS has years of experience with data management and cybersecurity, not to mention the means to oversee the collection and analysis of patient data on your behalf through our turnkey RPM program. If you think implementing RPM will be more trouble than it's worth, we're here to show you otherwise. Contact us today to learn more. Brenda Doles
