About us

Community Health Partnership of Illinois (CHP) is a Federally Qualified Health Center (FQHC) whose mission is to build and deliver quality, culturally responsive healthcare to our patients, caregivers and the communities we serve. CHP empowers individuals, including the uninsured, underserved, migrants and seasonal agricultural workers, to attain their best health and well-being. Our network is comprised of six health and dental clinics and multiple outreach sites across northern and central Illinois. Our services are designed to create access to quality primary care services for the uninsured and underserved. The vast majority, 99 percent of CHP patients, live in low-income households (less than 200 percent of the Federal Poverty Level (FPL)) with over two-thirds (67.2%) of families that live with poverty-level incomes (100% FPL). Three-quarters of our CHP patients do not have health insurance, which is in alignment with our mission that no one is denied services based on their inability to pay.

  • Dr. Eleace Sawyers Elected Board Treasurer of the NACHC Board of Directors We are thrilled to announce that on Sunday, August 17, 2025, during the National Association of Community Health Centers (NACHC) House of Delegates meeting at the 2025 CHI Community Health Conference & Expo, Dr. Sawyers was elected Treasurer of the NACHC Board. The NACHC Board of Directors consists of 8 elected officers, 2 representatives from each of the 10 regions, and 2 clinician representatives and two health center board members elected by the membership. Dr. Sawyers views this new role as an opportunity to fortify the foundation of our health center network and the communities we serve.   • Exciting News! Reopening of Our CHP Princeville Health Center After a brief hiatus, CHP is thrilled to announce the reopening of the CHP Princeville Health Center. We are eager to once again provide top-quality care to our beloved community. Dr. Wamaitha Sullivan will be the main provider at the site, with a dedicated team that will be supporting the operation. Also, we are reconnecting with all the companies, partners and other stakeholders in the community to make sure that we provide care to all. By reopening our doors, we're reaffirming our commitment to the health and well-being of Princeville residents. By providing accessible, high-quality care, we're bridging gaps and fostering a culture of wellness that benefits everyone.   • Community Health Center Leader Honored for Advocacy at NACHC Policy & Issues Forum BETHESDA, MD – The National Association of Community Health Centers (NACHC) has honored Dr. Eleace Sawyers President and CEO of Community Health Partnership of Illinois with the 2025 Elizabeth K. Cooke Advocacy MVP Award. This award recognizes advocates’ dedication and mobilization efforts to generate public and political support for the nation’s Community Health Centers. Dr. Sawyers was presented the award during NACHC’s 2025 Policy and Issues Forum (P&I) in Washington, D.C. Eleace is an accomplished healthcare executive with over 23 years’ experience in the health care industry. As the President & Chief Executive Officer for Community Health Partnership of Illinois (“CHP”), she oversees an interdisciplinary staff, providing primary preventative healthcare services from seven (7) health center sites serving patients from more than seven counties in Illinois. She is a proud recipient of numerous awards, including the 2023 Crain’s Chicago Business Notable Leaders in Health Care, the National Association of Community Health Centers’(NACHC) 2022 Outstanding Migrant Health Public Service Award, the Nonprofit HR 2022 Social Impact Woman to Watch Award, the 2017 Who’s Who in Black Cleveland “Game Changer” Award, the 2016 Community Development Leadership and Supporter Award (Diaspora Arts Coalition), the 2014 Florida Association of Community Health Centers Partnership Award and the 2012 South Florida’s 100 Most Accomplished Caribbean American Award. Eleace serves on many community and professional boards, is a graduate of Florida International University, Millsaps College and Walden University. She is certified in healthcare compliance, a member of the Health Care Compliance Association and a member of the American College of Healthcare Executives. Health centers are the largest primary care network in the U.S., serving 32.5 million people at more than 1,500 sites nationwide. The network of health center grassroots advocates consists of doctors, nurses, CEOs, frontline staff, board members and patients who have dedicated themselves to advancing equal access to outstanding healthcare in rural, urban, and frontier communities. “These honorees consistently go above and beyond to foster a culture of advocacy within their organizations and their health center,” said Kyu Rhee, MD, MPP, President and CEO of NACHC. “Their efforts to inspire and mobilize others are essential to building hope in a future where all communities nationwide have access to quality and affordable healthcare." Advocates are essential to the sustainability of health centers, which have saved countless lives since they were established 60 years ago. Health centers depend on bipartisan support from Congress and are grateful for this decades-long partnership.
